contracted with them..
PLEASE NOTE: I may be listed as "In-Network" or a "preferred provider" for your insurance, BUT each insurance policy is different with its
limitations and exclusions, deductible requirements, coinsurance, etc. Nutritional counseling or medical nutrition therapy MAY NOT be
covered on your plan.
If MNT is covered they may further limit coverage to certain diagnosis codes or excluded diagnosis codes, or may only cover specific number
of hours or visits per year.
Example: Medicare currently ONLY lets you see a dietitian for diabetes and kidney disease and ONLY IF your MD sends a referral AND you
can ONLY see a dietitian for 3 hours for the first year and 2 hours for the second. See how that works?
PLEASE Check with your insurance company or human resource department if you aren't sure if nutrition counseling is covered.

Are you wanting to pay for your dietitian appointment using your health insurance? No problem. MOST OF THE TIME (more than 80% of the
time) insurance companies pay for dietitian services as a prevention service that does not trigger the deductible, co-share, or copayment. BUT
THAT IS NOT A GUARANTEE.
We will bill your insurance company for you. However, if your insurance company denies the claim (regardless of the reason), or there is a
deductible, copayment, or co-share due, you will receive an invoice for the services you received.
PLEASE NOTE: Due to time constraints, our office can no longer verify eligibility and benefits with your insurance company as a courtesy for
you. Your insurance company has provided and extensive explanation of benefits manual when you enrolled.
By requesting an appointment with the dietitian, you are hiring a dietitian and you are accepting financial responsibility for the Medical Nutrition
Therapy services you receive. The dietitian begins working for you when they begin to professionally review the forms you submitted and begin
to prepare for your appointment, PRIOR to your appointment. If you later decide you don't want to see a dietitian and no show or cancel with less
than 48-hours notice, you will receive an invoice for $35 to reimburse the cost of the dietitian to review your forms.
